I don't know if this is possible but I need a module for joomla exactly like the one on this website:
www.vmgt.com
(The one to the far right)
I want people to be able to come to the website and enter that information, send it, and it will be sent to the back-end of Joomla. Also, when one has been submitted I need it to notify an e-mail address. This information needs to be recorded to the back-end of Joomla in some shape or form.

The only problem is it does not support multi options on the forms.
I need them to be able to pick locations.

- Letterman is great! The only problem is I am going to need the option of a drop-down menu. Does anyone know enough about editing modules to help me with that?

If you have Dreamweaver, you build the form using that quite easily.
