Right, I'd like to create a custom submission for that could include images or links to video and maybe 6 to 10 fields where questions can be answered.
I'd like also some checkboxes as well as emails to be sent when submitted.
The reason I'm putting this here is to see if anyone has any suggestions on what extension to use to make this happen.
It would also be important that I could style the submission form ...
