Module Positions Disappearing

Hi All,
My apologies if this thread is in the wrong section of the forum. If so, please feel free to move it.
I am currently dealing with a very serious issue that happened to get caught a couple days after our company website has gone live. To be brief, when someone submits the form through our contact us page, the left and right module positions disappear. To be more precise, it isn't necesarily due to the form submission, but a result of the URL change. I checked the page source and they aren't even being generated.

I have been working on this for quite some time so far today with no results. Any assistance you all could provide would be much appreciated.

- Did you assign those modules to all pages?
- Please reply with a direct link to the issue & create a new thread for each new issue.
