Popup compatible with JomSocial?

Looking for a modal popup extension that's compatible with JomSocial. Basically, it will be an opt-in form with a free report/ebook that will also automatically register users in the JomSocial component of the site.
Specifically, I need :
1. Should be able to set a "delay" as to when the popup appears to user.
2. When visitor enters "name" and "email address" they get access to the download and are also
automatically registered as a user in JomSocial.
3. Popup should be configurable in terms of styling so as to allow colors,etc. to match the sites design.
4. When popup is visible the background should be blurred out.
5. Should have cookie configuration so the visitor doesn't keep seeing an annoying popup as they
move from page to page.
6. Must be compatible with Jooomla 2.5+
