- I'd like to configure my website so that users must receive my approval before they can be registered. Is that possible? Is there an extension for that?

You do not need a special extension for registered user setup.
Joomla! User setup: Site > Global Configuration > System > User Settings
Allow User Registration: Set to NO. If set to YES, users will be allowed to self-register.
There are also additional settings in User Manager.

The users are part of the Joomla! core setup, if this is what you mean? So, no, the types of users and levels of access are set in the Joomla! core.
Types of Users: There are backend users And frontend users with varied levels of editing permissions.
Access Level: Public, Registered and Special
If you go to the Joomla! help file within the interface it gives a pretty clear explanation of access levels and type of users.
