I try to duplicate the “Error” System Outline with auto-generate Title. Success message but no new Error Outline ?????

You should not duplicate system outlines - nothing will happen. But I have raised a ticket about this as i'm not sure we should even show the "duplicate" button for "system outlines" at all

The Outline is actually getting duplicated but in the Users list

In the "outline" page - the top half is "user" outlines the bottom of the page is headed "system" outlines. The answer I gave you is the solution to your requirement. There is no point in duplicating a "system" outline since it will not behave as such.
