making a form for users to fill out that they can see updates online?

Basically, we sell water filtration equipment. We have a form that people can send in with a sample of their water to have us do a free water analysis so that we can recommend a system to them that will make their water better. Right now we have a pdf of a form that they can print out, fill out, and mail in with their water sample. Then we call them and discuss the results with them. I am trying to make a form they can fill out online and be able to check the results online sort of like how a support ticket system would work. I made a form with phil-a-form, and the form came out pretty well. I have not been able to figure out how I can make it so that they can check back later and see the results though:( Basically right now, the best I have been able to do is make a form that they can fill out online, then when they finish they submit the form and we get emailed the submitted form, they get it emailed, and it displays the submitted answers on their screen so they can print it out and mail it in. That a little better, however we would like to be able to have them submit the form, get a number assigned to them that they can just right on the water bottle they send in, and be able to log back on some time later and check the results of the water test and see our recommendations. In the long run, we would also like to be able to have a system set up for the people we have testing the samples, so that they can just type in the number on the sample and it will automatically update the status of the water test online to "received". Then when the customer checks online, they can see that we received the water sample and will be posting the results of the test as soon as we finish testing it. then just click the free water test link there, and it will bring up the form. If you look next to the free water test link, there is a View water test results link. Unfortunately though, like I siad, I have not been able to figure out how to set that up. Any suggestions would be greatly appreciated:)
