I've been there. Had this happen with a site almost 4 years ago. I used an untested extension, which opened up my entire website to this sort of malware.
If you've been using Akeeba Backup, and you know when the problem started, use a backup from before that date. That's the easy, brute method of solving the problem.
The other route is to do code compare, and run other diagnostics.
