{SOLVED} Contact Form with Photo Upload

- I'm looking for suggestions for a contact form with photo upload capability. Basically it would be general info, name, number, etc...and then a button to upload a photo submission. Can anyone suggest a nice solution for this? Many thanks in advance! =)
