No " insert " button in media lightbox...

I have Joomla 1.5.23
I see no insert button in the media light box when I try to insert a media file to an article.
If I use kepri, I have to scroll all the way to the right because I have long file names. Could this be the problem or is am I overlooking a setting?
I have attached a snapshot to illustrate missing insert button.

- Is this issue specific to a browser? Seems to work fine for me in Chrome / Safari / Firefox
