more familiar with html... but I can stumble my way around php...
Looking mostly for advice on which direction to take it... I thought maybe someone had done something like this before and would shed some light on a proven way to do it...
I would welcome any help if someone wanted to offer it, but I am not forward enough to ask someone to "donate" too much time on this subject.

and create a custom form type page
