Admin Password Failed

My Admin Password (admin and other user) is blocked and I can not get into joomla. I have searched this forum and the joomla forum and the standard ways to recever the password do not work. Can anyone suggest a way to get in?
What could have caused the password to fail? A hack? How do I prevent this?
Please help!!!

Just note that with any of these you need to translate your password into MD5.
