stupid question:
I edited one menu item created from one RT template and the item points to a page "xxx/index.php/page/mypage"
I created a new menu item (same type as as above) but now it points to a page "xx/index.php/mypage"
I don't understand why the demo one as the extra "/page" directory in it.

Joomla provide a "standard" htaccess file called htaccess.txt that you can use as the basis of a working .htaccess file - then you can add additional statements (like the ones you currently have in your .htaccess file) to that.
So, please update your .htaccess file to meet the requirements of Joomla first then I can check again.

didn't know it.
I updated it with the file data mentioned here:
docs.joomla.org/Preconfigured_htaccess
But now the site doesn't work at all.
Where I can find the correct htaccess file ?

ok so "portfolio" is a child of the "pages" menu item and so the SEF link will be /pages/portfolio whereas your shop menu item is a child of the root and so the SEF link will be /shop.
Does that explain it? btw to get rid of the "index.php" in the link turn on "url rewriting" in global configuration (provided you are now using the joomla htaccess).
