Clean-up Joomla Installation

I wanted to know if there is any way I can clean up my files and database, or if I need to. I have been running Joomla for a couple years and have used multiple templates, extensions, modules, etc. that I no longer use. I also have several menus, menu items, etc. from the various structures.
Should I be worried about all that stuff clogging up my DB and files?
