Looking for a form to upload images

I was looking at some of the extensions here but none seem to offer an upload feature within a form. I have been using breezingForms with good success but I have a need to rename the file that is being uploaded but have given up on their support. I have a form with three different sections and each section will allow for the user to upload an image. I need each image to be renamed using a concatenation of certain fields in the form.
Anyone have any ideas of what I might use to accomplish this. I have very little to no php coding experience.

I use fabrik extension for forms, ti allows file upload and you can limit the file types and sizes etc...
Its very good and lets you do most things and also has a great support forum.
