Fresco template not showing up in Template Manager Styles

After a successful install of the Fresco template I am not seeing the template "rt_fresco" in Template Manager: Styles. An update was available for Gantry which I installed (version 4.0.5).
The template is shown in the Template section of the Template Manager but when I click on Preview the following errors are shown in PHP's error log:
PHP Notice: Undefined index: in C:\<install_dir>\libraries\gantry\gantry.php on line 126
PHP Notice: Trying to get property of non-object in C:\<install_dir>\libraries\gantry\gantry.php on line 207
PHP Fatal error: Call to a member function get() on a non-object in C:\<install_dir>\libraries\gantry\gantry.php on line 267
PHP Fatal error: sqlsrv_errors contains an invalid type in C:\<install_dir>\libraries\joomla\database\database\sqlsrv.php on line 677

First, I have moved this thread to the Fresco forum. In the future please post to the template or extension related forum.
In answer to your issue. Have you assigned the Fresco template as the default template? If not, you need to do so.
If your issue is something more serious or server related, check the following.
SERVER REQUIREMENTS
We try to ensure that any Gantry template and the Gantry Library specifically will work with any modern and secure server environment. The recommended minimum requirements are:
PHP 5.2.17+ (5.3+ recommended)
MySQL 3.23 (5+ recommended)
Apache 1.3 (2.2+ recommended)
mod_mysql
mod_xml
mod_zlib
PERMISSIONS
Have you checked the permissions? Site >> System Information >> Directory Permissions
All should be green. Files should be 644 Folders 744. Check ownership as well.
