We have a client on J3.4.5 with latest Gantry and version rt_solarsentinel
The template's index.php keep getting hacked. All addons are up to date.

1. Make sure all passwords are long and random characters (minimum of 10 characters)
2. What security products are you using? I highly recommend Akeeba Admin Tools Pro in which you should use htaccess maker and administrator password facility. Alternatives would be rsfirewall. In other words, do not rely on joomla security alone you need to supplement that with a good security product.

Our passwords are secure + we use RS Firewall.
It has happened twice now.

Ok well I can only tell you to make sure that every RT product is up-to-date too. We've not had any instances of this being reported by anyone else. Personally I think Akeeba Admin Tools Pro is better that Rsfirewall but of course the choice is yours.
