Looking for form that will display user's historical results

I've been searching and haven't found an option that would allow users to fill out a form and see their current and past results.
It would be used on a health related site and the form or survey could be considered a journal. The user would fill in the form to specify how they are feeling, what they've eaten, external stimuli, etc.
I want the user to track their results in order to troubleshoot the health issues, then possibly integrate a charting tool like Plotalot. Anyone know of an extension that will show past results of a form or survey for a specific user?
