External Survey HTML?

Hi, I'm at a TOTAL loss for how to accomplish something. A survey has been written for our members using one of those online survey sites. We want the actual survey-taking to be on our site, though the data refers back to the survey site. The HTML code has been delivered to me in the form of an HTM file, but I can't figure out how to place on our site (see attached). I've tried two things with no success:
1. I used an HTML module, copied and pasted the code, and published the module to a menu. It kinda works, but the questions and response boxes and buttons aren't showing up.
2. I uploaded the HTM file and created a Wrapper to link to it. All I get is 404 error message or File Not Found message.
Can anyone help?
