need more mail / contact fields

I am designing a site for a music booking agency.
In the Contact / mail section there are four fields:
enter your name
enter your address
Message subject
Enter your message.
I would like to add some more one line fields.
Enter musicians name/ solo / duo band
Enter Date
Enter place
Enter approx start time and amount of sets
Pa required
Food and drink provded yes or no (maybe a box to tick/cross/ click)
Accomadation Provided yes or no (maybe a box to tick/cross/ click)
Where can i play around with the html and how do I make it so all fields must be filled in before the mail can be sent.
Also I need to do another one where people have to send an attachment, maybe that's too ambitious
Anyone come across this request before or can help with my quest???

- You could create your own form, for example with FacileForms or perForms .

Unlike Neil I don't need a complex form... however I would like to translate stuff like "Enter your name:" into another language. Is it possible to access that information somewhere and edit it?
Or is that where I need to somehow add another language to my Joomla install? If it's the latter I'd appreciate a link to those sites. Though I found Joomfish I haven't succeeded in finding the languages themselves.

All of this makes me wonder... is there a French.php file (for example)? If so, where do I get it? My searches to date have led me to stuff like JoomFish... but that just attempts to translate content (usually *rough* translations), right?
What I really wish is that someone could come to my site and click the language they want to use and BOOM... from there out the contact form, etc. is in that language.

PS I found that French.php files do exist... but didn't achieve my goal.
2 things that didn't work:
1) importing the php file via the Language Manager
2) uploading the French.php file into the languages folder and renaming it English.php
So... all that to say I'm trying but still haven't achieved the goal.
