Microsoft Access survey Help need it!!!

Hello there,
I am in urgent need of some heavy help for a Microsoft Access survey Form and Table. Please refer to the image I have attached. As you will see I am talking about a survey form I need to create but I am not able to create the table for it. In other words I can do the FORM all fancy-shmancy but I can not be able to make it connected to an access data base.
Can someone with Access abilities help me out with this little survey?

Not an answer, but a suggestion....
Rather than use Access for a survey, why not a third party component?
(Last time I checked, Access databases had limited simultaneous connections)
Doesn't even have to be a Joomla component, there are lots of free solutions that you can wrap...

Thank you but I am trying to do this for where I work and an outside resource it is not allowed. It has to be in M. Access.
